14.3 mill. The hospital and caregiving sectors lack a coherent solution to effectively support patients with multimorbidity and their relatives on integrated care pathways. The primary challenges are poor organization and inefficiency in communication and coordination between patients, relatives, and healthcare professionals. Consequently, patients and relatives are burdened with the responsibility of managing tasks, sharing knowledge, repeating information, and ensuring coordination.
In response, the I-PARD project has been initiated to improve integrated care pathways for patients with chronic illnesses and their relatives through the implementation of a digital platform solution. The objective is to empower patients and relatives to better handle their own health, leading to improved quality of life for patients with multimorbidity and their relatives.
The project develops, tests, and validates a digital solution that, to begin with, will be accessible to patients with Parkinson's disease, acquired brain or spinal cord injuries, their relatives, and healthcare professionals. By enabling coordination, communication, and collaboration across the hospital and caregiving sectors, the project seeks to facilitate improved healthcare experiences for patients and their relatives. Moreover, the project engages stakeholders from various domains, including the formal research community and different sectors, to ensure the most beneficial advancements in integrated care pathways.

12.2 mill. Healthcare systems worldwide face significant challenges due to shortages of healthcare workers (HCWs), and burnout due to bad work environments. While new hospitals are being built with single-bed patient rooms to ensure privacy and reduce patient infection risks, this architectural design poses additional problems. The physical and mental distance between HCWs and patients increases, and HCWs must oversee more rooms, resulting in fewer patient check-ins. Consequently, patients feel uninformed and isolated, and the risk of adverse events such as falls increases.
iAware is an AI-trained workflow technology that prioritizes the well-being of HCWs AND patients. It introduces a virtual "window" that allows patients to stay informed about relevant HCW activities and vice versa. By virtually removing the barriers between patients and HCWs, iAware reduces the need for frequent calls from patients and colleagues, enabling HCWs to work more efficiently.
The two-way exchange of information is facilitated by using existing tablets and screens in hospitals. iAware's integrable solution incorporates predictive ward rounds to ensure timely patient visits, patient movement advice, and automated registrations of cleaned and available rooms for optimized room utilization.
This approach enables real-time workflow optimization intending to improve the efficiency and the satisfaction of patients and HCWs.
